I must confess I don't have a u2R installed here, and I can't have one 
easily, but :

1/ I'm not sure you set the outputs in the Win-Test Options / 
MKI/MKII... configuration dialog *or* was it in the Microham router 
application ?

2/ If the settings are from the Win-Test dialog, you must also enable 
the "Protocol controlled" option in the Microham router (probably in the 
Acc tab, if any).

I checked the U2R users manual (v8 - March 1st 2016) - I guess it's the 
latest - but I couldn't find any reference to the Acc part of the "PTT & 
Acc" Tab :-(
